Mack 36, of Phoenix; assigned to the 3rd Battalion, 21st Infantry Regiment, 25th Infantry Division, Fort Lewis, Wash.; killed Jan. 13 when an improvised explosive device hit his military vehicle in Mosul, Iraq.
