How AT LLC Works

Using a sample of the original document to be processed, including other client information and output record format, AT LLC develops the perfect integration of your form and our programming to handle the data input, output, quality management and project management. Note, design and implementation of a new form / document format for greater statistical compilation, is always available.

Next, AT LLC will enter a test sample of the specific document(s), output the data and provide it to the client for review and acceptance. Once this data output meets with the client's approval, AT LLC and the client will set up a start date, data upload schedule and shipping schedule for the original documents to start arriving and begin processing.

Upon client approval, AT LLC assigns an account representative so any and all questions may be answered on the first call without needing to speak to a different person from each area in question.

Much of our data management work begins in our Document Preparation Department where opening, extracting, sorting and coding is done prior to data capture. Thousands of documents pass through the hands of our staff each day! Our Document Prep staff additionally processes exception items.

Manually processed and electronically captured data is then transmitted to our verification department where various edit checks, data validation, and database management functions are performed. Our verification department provides application design and development as well as statistical and analytical reports. We can develop a special data entry "Table & Rules" for each client's document(s), with fill validation, precise error checking features and output design based on each client's in-house record format requirement. It is also possible to include any client specific data audit feature, including range checks, logical date checks, batch balancing, record balancing, check digit calculation, table look-up and extraction. In addition, specified fields can be limited to accept alpha, numeric or a combination of both. Tables and Rules can be defined to insure accuracy and trap erroneous information.

At AT LLC we can customize programming to any record layout to ensure uploading the client specific format to their system. We may also assist in the development and implementation of programming, reports and other data specific requirements your company may have.

AT LLC processes Hcfa 1500, UB92 & ADA forms daily by scanning/imaging these documents or accepting them from our clients via ftp and either OCR or manual double key & verify process. The use of our key-from image technology will allow us to route non-readable (ocr) forms to this system where we will double key from images any unrecognized data. The output data is formatted as electronic claims which are transmitted to our clients in an ascii comma delimited, flat ascii, 837 X12 or NSF format (or any other format they might require) for processing.

 

We can process individual HCFA and institutional UB92 claims as well as dental claims. In the mailroom, the claims get opened, removed from their envelopes and sorted into separate piles: single sheet claims, multiple sheet claims, HCFA's, UB92's, Dental Claims, and miscellaneous other sheets which cannot be processed and quality level of each form. We will not process a document which does not contain a claim form, therefore, any claim or correspondence, whether single or multi-sheet, must always contain a claim form.

 

The forms are scanned at 200-300 dpi depending on the quality of the form coming into process. During the time pages are scanned the fields are OCR'd and validated for field content. If acceptable, a control number is assigned. The pages are then imaged in a tif format and batched. While scanning we use a red filter located in the scanner to remove the form. As a result, our images are approximately 15k or so each. Critical to our process are two exception handlers. This exception handler allows the system to determine if the claims are good enough to OCR ( any claims that do not meet validation rules or that do not OCR at more than 80% accuracy go to this exception) if they are not the claims is re-routed to our exception program where the claims will be keyed from image.

 

Why 80%? Well we found that it is more effective for an operator to key all the data if more than 20% needs to be corrected. If there is more than one character wrong in a field it is quicker for our operators to re-key the whole field than try to fix it. Statistically, a more than 20% error rate means that most fields will contain more than one error.

 

Once the forms are scanned and OCR'd successfully they get assigned a control number and an image number which is indexed in the output file associated with that claim. The images get stored on our Windows NT Server where we will keep them their for our clients to access them through our online search or where they will be transferred to a CD-ROM or Drive of their choice. The rejected pages are either re-scanned as images or re-routed to our exception key entry operators. While this is going on, our key entry stations are looking for batches of images that need to be corrected or key entered. The batch is opened on a verification station creating a split screen displaying the image on the right and the zoomed in zone on the left and key entry fields below with any unreadable characters displayed by highlighting it in yellow.

 

Once the batch is complete, and the image record is flagged as keyed, the automated job-flow process sends the batch through the output queue into a text file holding the job in an ascii comma delimited format. At the end of the day we generate a transmission report with the file which details each of the batches which were processed.

Quality Assurance and Accuricy

AT LLC sees quality and data accuracy as being of great importance. We guarantee our clients the highest level of accuracy to 99.97%. We do this by maintaining stringent checks on client data at all times during the keying process. These checks form part of a company-wide standard operating procedure, which is always followed. As a result, AT LLCI is able to achieve and maintain a higher accuracy rate on single key entry work than many accomplish with double key entry. In the area of quality control, we have specific edits and validation tables that are customized by our experienced programmers for each job. Simply put, validation routines are algorithms that examine ICR results for reasonableness against predetermined standards.

 

We have three basic types of validation routines:

 Table look-up: one or more fields are validated against a database. If there is an exact match the odds are good that the data is correct since it proved to be congruent with a set of known quantities.

 Data/Range checks: the field or character information is compared with a model to see that it conforms to a specific type, such as a specific alphanumeric sequence or date, such as the date of purchase on a bill of sale used to support a title guarantee application. A validation routine might also check to see that a number is within a specific range or contains a certain number of digits.

 Relationship validation: when a column of numbers totals correctly. If there is a match on the total, then the probability is extremely high that all the numbers are correct. All data fields are systematically analyzed to ensure data validity and integrity. Among the variables that the quality control system check: Field Length, Data Type, Relational Data Integrity, Valid Codes, Applicable Abbreviations, and when applicable, Mandatory Fields, & Sequential Numbering.

All processing is overseen by experienced on premises managers. They are responsible for the final quality assurance of data. Special quality control checks designed specifically for each project are utilized in addition to the quality control procedures that are in place during processing. These supplemental checks, coupled with professional account management, further ensure data integrity, quality, accuracy and dependability.

Hard copies can be stored for a period of three months. After the completion of a project, if required, AT LLC will store clients' hard copy originals at no charge for a period of three months on a rotation basis, whereby as the new hardcopy arrives the processed hardcopy will be forwarded. AT LLC will consult with the client regarding the return of records to the client's designated address or the destruction of any unwanted records. AT LLC can place all records on to CD-ROM (compact diskette read only memory) in a *.tif, dbf, mdb or text format base on customers?need for retrieval purposes, thereby reliving the need to store hard copy documents

OCR

We have expanded our traditional methods of data capture through scanning, optical character recognition (OCR), intelligent character recognition (ICR) and optical mark recognition (OMR) technologies. These automated data capture methods are ideal for high speed processing of a large volume of identical forms such as: Insurance, Medical, Banking, and Bill Remittance Forms.

OCR is a process of using specialized software to turn images into editable text. Together, they provide us the ability to turn paper documents into re-usable formats, like MS Word files, MS Excel files, PDF files or HTML pages. OCR can also be used to turn image-based PDF files back into editable text. Having been used to such automated methods of document processing provides its OCR perfection services in various formats such as: Checks with OCR, Full text with OCR, Indexing using OCR, Barcode recognition, Handwritten text with OCR. For better quality of OCR'd file, we do proof reading. Files with normal text undergo online proofing whereas texts containing special characters, chemical formulae, superscript characters, subscript characters that are more difficult to OCR through software, require manual proof reading. This is done through comparing hard copy with soft copy.

ICR

Handwriting is a natural form of communication that people have used for centuries; and there are vast areas where keyboard and keypad entry simply never work as effectively as natural handwriting. ICR (Intelligent Character Recognition) enables handwritten text to be extracted and used for automatic indexing of images.

Indexing

AT LLC shall provide various types of indexing database as per clients requirement. For this particular requirement we have to OCR the scanned images and sort the data to build the database as per requirement.

The forms are scanned at 200-300 dpi depending on the quality of the form coming into process. During the time pages are scanned the fields are OCR'd and validated for field content. If acceptable, a control number is assigned. The pages are then imaged in a tif format and batched. While scanning we use a red filter located in the scanner to remove the form. As a result, our images are approximately 15k or so each. Critical to our process are two exception handlers. This exception handler allows the system to determine if the claims are good enough to OCR ( any claims that do not meet validation rules or that do not OCR at more than 80% accuracy go to this exception) if they are not the claims is re-routed to our exception program where the claims will be keyed from image.

 

Why 80%? Well we found that it is more effective for an operator to key all the data if more than 20% needs to be corrected. If there is more than one character wrong in a field it is quicker for our operators to re-key the whole field than try to fix it. Statistically, a more than 20% error rate means that most fields will contain more than one error.

 

Once the forms are scanned and OCR'd successfully they get assigned a control number and an image number which is indexed in the output file associated with that claim. The images get stored on our Windows NT Server where we will keep them their for our clients to access them through our online search or where they will be transferred to a CD-ROM or Drive of their choice. The rejected pages are either re-scanned as images or re-routed to our exception key entry operators. While this is going on, our key entry stations are looking for batches of images that need to be corrected or key entered. The batch is opened on a verification station creating a split screen displaying the image on the right and the zoomed in zone on the left and key entry fields below with any unreadable characters displayed by highlighting it in yellow.

 

Once the batch is complete, and the image record is flagged as keyed, the automated job-flow process sends the batch through the output queue into a text file holding the job in an ascii comma delimited format. At the end of the day we generate a transmission report with the file which details each of the batches which were processed.

Quality Assurance and Accuricy

AT LLC sees quality and data accuracy as being of great importance. We guarantee our clients the highest level of accuracy to 99.97%. We do this by maintaining stringent checks on client data at all times during the keying process. These checks form part of a company-wide standard operating procedure, which is always followed. As a result, AT LLCI is able to achieve and maintain a higher accuracy rate on single key entry work than many accomplish with double key entry. In the area of quality control, we have specific edits and validation tables that are customized by our experienced programmers for each job. Simply put, validation routines are algorithms that examine ICR results for reasonableness against predetermined standards.

 

We have three basic types of validation routines:

 Table look-up: one or more fields are validated against a database. If there is an exact match the odds are good that the data is correct since it proved to be congruent with a set of known quantities.

 Data/Range checks: the field or character information is compared with a model to see that it conforms to a specific type, such as a specific alphanumeric sequence or date, such as the date of purchase on a bill of sale used to support a title guarantee application. A validation routine might also check to see that a number is within a specific range or contains a certain number of digits.

 Relationship validation: when a column of numbers totals correctly. If there is a match on the total, then the probability is extremely high that all the numbers are correct. All data fields are systematically analyzed to ensure data validity and integrity. Among the variables that the quality control system check: Field Length, Data Type, Relational Data Integrity, Valid Codes, Applicable Abbreviations, and when applicable, Mandatory Fields, & Sequential Numbering.

All processing is overseen by experienced on premises managers. They are responsible for the final quality assurance of data. Special quality control checks designed specifically for each project are utilized in addition to the quality control procedures that are in place during processing. These supplemental checks, coupled with professional account management, further ensure data integrity, quality, accuracy and dependability.

Hard copies can be stored for a period of three months. After the completion of a project, if required, AT LLC will store clients' hard copy originals at no charge for a period of three months on a rotation basis, whereby as the new hardcopy arrives the processed hardcopy will be forwarded. AT LLC will consult with the client regarding the return of records to the client's designated address or the destruction of any unwanted records. AT LLC can place all records on to CD-ROM (compact diskette read only memory) in a *.tif, dbf, mdb or text format base on customers?need for retrieval purposes, thereby reliving the need to store hard copy documents

Data Entry and Processing

We provide data entry and processing services to clients. Data Processing essentially involves collecting data, creating a master data, tabulating available data, validating the data as per given rules, creating a database and transferring the output back in acceptable media. In the interest of quick and accurate retrieval, some field based indexing is recommended. OCR

We have expanded our traditional methods of data capture through scanning, optical character recognition (OCR), intelligent character recognition (ICR) and optical mark recognition (OMR) technologies. These automated data capture methods are ideal for high speed processing of a large volume of identical forms such as: Insurance, Medical, Banking, and Bill Remittance Forms.

OCR is a process of using specialized software to turn images into editable text. Together, they provide us the ability to turn paper documents into re-usable formats, like MS Word files, MS Excel files, PDF files or HTML pages. OCR can also be used to turn image-based PDF files back into editable text. Having been used to such automated methods of document processing provides its OCR perfection services in various formats such as: Checks with OCR, Full text with OCR, Indexing using OCR, Barcode recognition, Handwritten text with OCR. For better quality of OCR'd file, we do proof reading. Files with normal text undergo online proofing whereas texts containing special characters, chemical formulae, superscript characters, subscript characters that are more difficult to OCR through software, require manual proof reading. This is done through comparing hard copy with soft copy.

ICR

Handwriting is a natural form of communication that people have used for centuries; and there are vast areas where keyboard and keypad entry simply never work as effectively as natural handwriting. ICR (Intelligent Character Recognition) enables handwritten text to be extracted and used for automatic indexing of images.

Indexing

AT LLC shall provide various types of indexing database as per clients requirement. For this particular requirement we have to OCR the scanned images and sort the data to build the database as per requirement.
